1) READ this entire page
2) Print out BOTH the price page & the Crossroads Consent form
3) Circle, mark, write on prices page what you will be electing be done for your pet(s)
4) Fill in & sign Crossroads consent form
5) PLEASE include your name, address, phone number, pet's name, emergency contact, & vet info, if you have one, with these forms.
6) INCLUDE your payment with check made out to Humane Society of Lancaster, SC
Reduced cost Spay/Neuter information:
Our sincere apologies - but the prices have increased to us from the vet so we must increase our prices to you. Please look at the new prices, still great prices!
We are so thrilled to offer a reduced cost spay/neuter!! We are using Crossroads Animal Clinic In Lowrys! They are gracious enough to give a reasonable rate when we bring approx 4 pets at a time! We will be taking dates as they are available so please be patient, if you cannot wait for a date that we have and can provide your own transportation to Crossroads in Chester, SC (Lowrys) you may be eligible for a voucher by paying us the fees ahead of time and then making an appointment with them at your convenience. Pets MUST spend the night there for safety reason after surgery. We will leave from Lancaster early morning approx 7:30am to take the pets down, then pick them up the next day around lunchtime to return to Lancaster approx. 1-2pm. **Times subject to change**
**PLEASE NOTE a rabies tag is NOT proof of rabies vaccine, you must send in or bring with you at drop off time a COPY of the actual rabies certificate your vet provided to you or Crossroads WILL charge you for the new rabies vaccine.
Please read the consent forms from Crossroads vet and the price list for procedures. If you are interested in having more then one pet altered, then please print up and fill in one consent form for each pet.
If you still want to do this after reading these then please , send me the filled in and signed consent form along with a check in the full amount. Also please make sure to include your full name, address and phone along with the name, age, sex and breed of pet and approximate weight.
At the very top of the Consent Form please write in the breed of dog or cat and a brief description such as the pets coloring or any special markings.
Once we receive your payment I will then call you to set it up for you . Also please know that a valid Rabies certificate is required, if pet has not had a Rabies shot in the past year, then the Crossroads vet requires they administer one at a cost of $5.00. Please enclose a copy
{ please do not send the original } of the Rabies Certificate given to you from your vet when dog was vaccinated.
As soon as I receive the filled out Consent Form along with a check in the appropriate amount I will book you and call to let you know what the date will be. The Vet has limited us to 4 pets per trip so the dates will book up very fast. I cannot hold a date unless the spay or neuter has been pre paid. If you book and for some reason must cancel you will be given the choice of a refund or a re booking at a later date.
Pick up will be 7:30 am in the parking lot of K Mart behind Arby's on Main St. Lancaster. Drop off will be the following day same place around 12:30 or 1:00. A phone number where you can be contacted on the drop off day is essential so we can let you know when we arrive back in Lancaster with your pets. If your pet is prone to being aggressive with strangers it is advised a muzzle be placed on the pet before it is put in the carrier. Having a strange person reach inside the carrier after riding in it for an hour can cause a pet to lash out and bite and we cannot put our handlers and drivers in a hazardous situation.
Please make sure to pick up food and water dishes by midnight the day before the surgery is scheduled. Pets MUST have an empty stomach when undergoing a spay or neuter.
Also it is advised that you have your pet inside a car carrier as placing unfamiliar dogs and or cats loose inside a car is not advisable as it can lead to dangerous driving conditions and pose a hazard to the driver and the pets. We do have a few small carriers suitable for cats but currently we only have one carrier large enough to accommodate a German Shepard or similar large breed dog, so we cannot guarantee we will have enough carriers of a suitable size for your pet.

Please consider having your pets micro chipped for identification purposes. If your pet should ever get lost or stolen a micro chip will identify them as belonging to you with your contact information nationwide. The micro chip price listed on the attached price list includes the cost for registering your pet in a nation wide data bank. Our cost is only $15.00 per pet, cost for the chip at a Vet would be $45.00 plus an additional $25.00 to register, so with us you save $55.00. Once you purchase the chip there are NO additional charges . NO monthly fee, NO charge for registering your pet, EVER!
****IF you do NOT have a carrier please ask us in advance if we can arrange to have one for use, if your pet is NOT in a carrier they cannot be transported. Please do not wait until transport morning.
****If you must cancel please be considerate and allow at least 24 hours notice so that we may get the next pet waiting to go time to prepare.

Voucher information: READ CAREFULLY PLEASE!
READ ALL INFO BELOW CAREFULLY! If you do not supply appropriate forms and information it will slow down the process and take longer, please follow instructions at top of page and below.
1) Check made out to Humane Society of Lancaster, SC must be included with ALL paperwork to receive your voucher.
2) DO NOT call Crossroads to schedule an appointment until you have received your voucher. Their address is 2501 Lowrys Hwy Chester, SC 29706 if you would like to see how far they are.
3) Vouchers are provided so that you may bring your pet at your available time, transportation is NOT included, you MUST provide your pets transportation with voucher use. If you cannot transport your pet please see above to be added to our transport list reduced cost spay/neuter.
4) In summary; For voucher use: Print out consent form, fill it out entirely, send check, all your pets information & what you would like done for your pet, to us at HSLSC c/o Mary Reimers 2007 Green Peach Road Lancaster, SC 29720. When you receive your voucher call Crossroads for an appointment time that is good for them, you, & your pet. Bare in mind the microchips are only the one time fee of $15 & come with FREE registration via web or mail! Comforting to know they can be identified without a collar :)! BRING THE VOUCHER TO YOUR APPOINTMENT!!
